Elizabeth Macshadiya is a psychiatric nurse practitioner with training in inner work, acceptance and behavioral approaches, motivational interviewing, and supportive psychotherapy, with additional focus on uncomplicated hormone replacement therapy for perimenopause, thyroid optimization, and the metabolic and somatic foundations of mental health.

  2. 2Meeting a Protective PatternA working relationship with the most active protective pattern. The inner critic, the perfectionist, the caretaker, the productivity driver.
  3. 3Working With a Reactive PatternThe pattern that takes over when overwhelm hits. The wine, the scrolling, the rage, the withdrawal. Listened to rather than fought.
  4. 4Stepping Back and ValuesA precise look at what you actually value, beneath what the loudest patterns have been demanding. Difficult thoughts become workable.
  5. 5Permission and ApproachWith consent from every protective pattern, you turn toward the tender younger aspect underneath. No pressure, no flooding. Just contact.
  6. 6Witnessing and ReleaseThe heart of the work. The younger aspect is witnessed, retrieved from where it has been stuck, and releases what it has been carrying.
  7. 7Reorganizing the SystemProtective patterns choose new roles. Inner conflicts soften. New patterns of inner relationship begin to install themselves into daily life.
  8. 8Committed Action and MaintenanceThe work becomes a life. You leave with a daily practice, a three month action map, and the grounded operating layer reliably online.

Schedule A for founding cohort 1 sets eight group sessions of 75 minutes by video, six to eight participants, with one maintenance session in February 2027.

What Self OS is not

Self OS is not therapy, counseling, psychotherapy, medical care, or psychiatric care. It does not assess, diagnose, or treat any condition, and nothing in it is medical advice. The professional background of your guide (a board-certified psychiatric mental health nurse practitioner) informs the education you receive; it does not create a clinician-patient relationship, and your guide will not act as your clinician within the Program. If you want clinical care, we will help you find a licensed clinician in your state.

The rest of it

Your health care remains with your own clinicians. You agree not to start, stop, or change any treatment or medication based on anything in the Program without talking to the clinician who manages it. If you ask your guide a question about a symptom, a medication, a supplement, a hormone, or a diagnosis, your guide will point you to a licensed clinician rather than answer it in the Program.

The Program does not provide crisis services. If you are in crisis, have thoughts of harming yourself or someone else, or need urgent help, call or text 988, call 911, or go to the nearest emergency room.

Frequently asked

Is this coaching or therapy?

This is coaching, not psychotherapy and not psychiatric care. The framework is appropriate for high functioning adults seeking developmental work. If clinical care is indicated at any point, Elizabeth will help you identify appropriate resources.

How long does the program take?

Eight sessions of sixty to seventy five minutes, typically held weekly or every other week. Maintenance afterward is optional and tends to be quarterly for the first year, then as needed.

Do I need to know anything about inner work in advance?

No. The model is introduced in plain language as we go. You will leave the program fluent in the working vocabulary, and with a daily practice you can carry forward.

Can I do this work while on medication or HRT?

Yes. Medication and hormone optimization stabilize the biological floor on which the inner work lands. Many clients find that as their system reorganizes, their relationship to their medications becomes clearer, which is a conversation to have with the prescribing clinician.

Will this bring up things I am not ready for?

That is a real concern and the model is built around it. We do not go to the wounded parts without permission from the protective parts. Your system sets the pace.

What if I have a trauma history?

A trauma adapted variant of the program is available, typically twelve sessions, conducted with explicit coordination with your trauma trained psychotherapist. Reach out and we can discuss whether this is appropriate for you.
